Apicoectomy of Lower Bicuspid

A 45-year-old woman presented with swelling and discomfort associated with tooth #29. Radiographically, a large radiolucent area was visible at the apex of tooth #29. A fairly new crown and cast core were present with a silver point root canal. Options: Attempt conventional re-treatment through the crown and cast core or apicoectomy. After discussing the treatment options with the referring doctor and patient, an apicoectomy was decided.
