Apicoectomy of Lower Bicuspid

A 45-year-old woman presented with swelling and discomfort associated with tooth #29. Radiographically, a large radiolucent area was visible at the apex of tooth #29. A fairly new crown and cast core were present with a silver point root canal. Options: Attempt conventional re-treatment through the crown and cast core or apicoectomy. After discussing the treatment options with the referring doctor and patient, an apicoectomy was decided.

Pre-operative x-ray

Root end beveled and prepared with ultrasonics

Canal sealed with super EBA cement

Post-op x-ray

6-month post op X-ray showing total healing

* Note: Conventional re-treatment would have been an option. It is always wise to attempt re-treatment whenever possible.
